چکیده

A 45-year-old woman presented to the emergency department with left wrist pain following a fall. Physical exam was remarkable for soft tissue swelling of the palmar and dorsal hand, point tenderness at the base of the second metacarpal, and limited active range of motion. Three-view wrist radiographs showed significant soft tissue swelling without evidence of fracture or dislocation. Computed tomography obtained due to concern for an occult fracture revealed a comminuted fracture of the trapezoid bone involving the articular surface with an anteriorly displaced bone fragment (Figures 1 and 2). The patient was placed in a sugar-tong splint and given orthopedic follow up. Fractures of the trapezoid bone are rare, accounting for 0.4% of carpal fractures. This is primarily due to its protected position in the wrist, nestled within strong intercarpal ligaments between the surrounding carpal bones and the base of the second metacarpal. Its position and integrity contribute vitally to the carpal arch, but make detection of injury elusive. Fractures are rarely isolated, often involving fractures of the second metacarpal base. Patients may have second metacarpal base tenderness, ‘‘snuffbox’’ tenderness, or pain with axial loading of the second digit after trauma involving an axial or bending force along the second metacarpal. The overlapping bones and intimate articulations of the wrist make plain radiographs difficult to interpret. In 2 separate reports reviewing 17 trapezoid fractures, only 1 fracture was diagnosed by plain radiographs; all others required advanced imaging. In 1 study, 30% of wrist fractures missed by plain radiograph were diagnosed by computed tomography. Treatment approaches have varied from cast immobilization to open reduction and internal fixation, depending on degree of displacement and integrity of the carpal arch. Unrecognized

Occult fracture of the trapezoid bone: a report on two cases.

The authors report two cases of isolated, undisplaced, fractures of the trapezoid bone. Because of its well-protected position in the wrist, traumatic lesions of the trapezoid bone are rare. Dislocation of the trapezoid has been reported in 30 patients; fractures are seen even less frequently and usually go with dorsal displacement. To our knowledge and based on a Medline literature review, onl...

Trapezoid and Triquetrum Fracture: Report of a Rare Fracture Pattern

Trapezoid bone fracture is the least common fracture owing to the fact that it is strictly bounded to scaphoid, capitate and the base of the second metacarpal. Isolated fractures are rarely seen and usually related to the high energy trauma. It is conservatively treated without dislocation. Triquetrum fractures are the second least common fractures among carpal bones. The body fractures are usu...

Trapezoid stress fracture in an international shot-putter: a case report.

Trapezoid fractures are relatively rare upper limb fractures. The case report of a trapezoid stress fracture in the dominant hand of a 22 year old international standard shot-putter is presented.

A rare combined injury of dorsal fracture-dislocation of four carpometacarpal joints and trapezium, trapezoid and distal radius bone fractures.

Incorrect or delayed diagnosis and treatment of the carpometacarpal fracture-dislocations is often associated with poor prognosis. We present a rare case of unusual pattern of injury, involving dorsal dislocation of four ulnar carpometacarpal joints, associated with fracture of the trapezium, a burst fracture of the trapezoid  bone and an extra-articular fracture of the third distal  of the rad...

Multiple carpometacarpal dislocations and an ipsilateral scapho-trapezium-trapezoid fracture-dislocation: a rare pattern of injury.

We report a rare case of simultaneous dorsal dislocation of 4 ulnar carpometacarpal joints and dorsoradial dislocation of the trapezium with an associated fracture of the scaphoid tuberosity. The injuries were diagnosed early and treated successfully with closed reduction and transfixation using Kirschner wires. The functional results were excellent at 17-month follow-up.
